Applciation Checklist
- Birth Certificate.
- Social Security card.
- Proof of address (any utility bill with your name, or a lease).
- Proof of income, using any one of the following:
- Four (4) consecutive pay stubs (of each person who works in the household)
- TANIF/Welfare Letter
- S.S.I. Letter
- Prior year's income tax return
- Notarized letter from your employer
- Immunization and Doctor's Physicals Records
- Health Insurance card
- Medical reports of any special needs, if applicable
- Dental Records
